We are seeking a dedicated and detail-oriented MDS Coordinator to join our team at RegalCare at Harwich. As a key member of our healthcare team, you will play a vital role in ensuring the highest quality of care for our patients and residents.
As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining accurate and up-to-date patient and resident records. You will work closely with our medical team, families, and other healthcare professionals to provide comprehensive care to our patients and residents.
Coordinate the MDS assessment process, including scheduling and facilitating assessments with interdisciplinary teams
Ensure accurate and timely completion of MDS assessments, RAPs, and other required documentation
Maintain accurate and up-to-date patient and resident records, including medical and social histories
Collaborate with medical teams, families, and other healthcare professionals to ensure comprehensive care
Develop and implement effective communication strategies to ensure seamless coordination of patient and resident care
Identify and address any discrepancies or inconsistencies in patient and resident records
Participate in quality improvement initiatives to improve patient and resident outcomes
High School Diploma or equivalent required
MDS Coordinator experience preferred
RN/LPN Nursing licensure
Strong organizational and analytical skills, with attention to detail and ability to work in a fast-paced environment
Excellent communication and interpersonal skills, with ability to work effectively with diverse stakeholders
Ability to maintain confidentiality and handle sensitive information
