Qualifications

• High School Diploma or equivalent required.
• MDS Coordinator experience preferred.
• RN/LPN nursing licensure required.
• Strong organizational and analytical skills with attention to detail.
• Excellent communication and interpersonal skills.
• Ability to maintain confidentiality and handle sensitive information.

Responsibilities

• Coordinate the MDS assessment process with interdisciplinary teams.
• Ensure timely completion of MDS assessments, RAPs, and documentation.
• Maintain accurate patient and resident records.
• Collaborate with medical teams and families to ensure comprehensive care.
• Develop communication strategies for seamless coordination of care.
• Identify discrepancies in patient records.
