Job summary

RegalCare at Harwich is seeking an experienced MDS Coordinator to ensure accurate and timely completion of assessments and to maintain up-to-date records for patients and residents. You will work closely with our medical team, families, and other healthcare professionals to provide comprehensive care.

As part of the role, you will coordinate the MDS process, drive RAPs and essential documentation, and support quality improvement initiatives in a fast-paced healthcare environment.

Qualifications

• MDS Coordinator experience preferred
• RN/LPN licensure required
• High School Diploma or equivalent required

Responsibilities

• Coordinate the MDS assessment process, including scheduling and facilitating assessments with interdisciplinary teams
• Ensure accurate and timely completion of MDS assessments, RAPs, and other required documentation
• Maintain accurate and up-to-date patient and resident records, including medical and social histories
• Collaborate with medical teams, families, and other healthcare professionals to ensure comprehensive care
• Develop and implement effective communication strategies to ensure seamless coordination of patient and resident care
• Identify and address any discrepancies or inconsistencies in patient and resident records
• Participate in quality improvement initiatives to improve patient and resident outcomes

About the Role:

As an MDS Coordinator, you will be responsible for coordinating the Minimum Data Set (MDS) assessment process, ensuring accurate and timely completion of assessments, and maintaining accurate and up-to-date patient and resident records. You will work closely with our medical team, families, and other healthcare professionals to provide comprehensive care to our patients and residents.
