Job summary

Lee Health & Rehab Center in Pennington Gap, VA is hiring an LPN MDS Coordinator to lead the MDS and care plan process, ensuring regulatory compliance and accurate reimbursement. The role supports quality improvement, staff education, and utilization management within a full-time daytime schedule.

The ideal candidate holds an active LPN license in Virginia, with 2 years' MDS experience and RAC-CT certification preferred. Competitive hourly pay and comprehensive benefits are available.

Qualifications

• Two years' experience with MDS process and/or RAC-CT certification preferred.

Responsibilities

• Coordinates and participates in the completion of MDS assessments, triggers, CAAs and comprehensive plans of care via review of the resident's medical record, communication with the Interdisciplinary team and with physicians and family.
• Completes portions of the MDS, CAAs, and Care plans to ensure timely and accurate assessment.
• Maintains a schedule for all residents and provides it to the care team to allow timely assessment completion.
• Ensures interdisciplinary assessments use appropriate reference periods and comply with guidelines.
• Attests to completeness of MDS through signature and supports regulatory compliance.
