Job summary

Metropolis Rehabilitation and Health Care Center is seeking a passionate MDS Coordinator | LPN to fulfill responsibilities related to the RAI process and care management from admission to discharge.

The ideal candidate will have a genuine desire to work with seniors and be licensed as an LPN in Illinois. Benefits include health insurance, 401(k), and tuition reimbursement.

Qualifications

• Must possess a thorough understanding of the Quality Indicator process and OBRA regulations.
• Minimum one year of experience in a LTC/SNF/AL/MC setting preferred.
• Excellent knowledge of Case-Mix and Federal Medicare PPS process.

Responsibilities

• Responsible for timely MDS completion and care management processes.
• Coordinate information systems operations with nursing leadership.
• Ensure compliance with Federal, State, and Certification guidelines.

MDS Coordinator | LPN

What Will You Do in This Role?

As a MDS Coordinator | LPN, you will be responsible for timely and accurate completion of both the RAI process and care management process from admission to discharge in accordance with company policy and procedures, and Federal, State and Certification guidelines, and all other entities as appropriate- Minimum Data Set, discharge, and admission tracking, etc. With direction from the Director of Nursing and VP of Clinical Reimbursement, you may coordinate information systems operations and education for the clinical department.

• Currently licensed as a Licensed Practical Nurse (LPN) in the state in which you are practicing
• Prior MDS experience highly preferred
• Minimum of one year of experience in a LTC/SNF/AL/MC setting highly preferred
• Excellent knowledge of Case-Mix, the Federal Medicare PPS process, and Medicaid reimbursement highly preferred
• Must possess a thorough understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
• Knowledge of the care planning process required
• Professional image in both appearance and behavior
• Excellent written and oral communication skills
