What Will You Do in This Role?

As an MDS Coordinator | LPN, you will be responsible for the timely and accurate completion of the RAI process and care management from admission to discharge, following company policies, federal, state, and certification guidelines, including Minimum Data Set, discharge, and admission tracking. Under the guidance of the Director of Nursing and VP of Clinical Reimbursement, you may also coordinate information systems operations and provide education for the clinical department.

Do You Have What It Takes?

• A passion for working with seniors
• A flexible, energetic, and positive personality
• Currently licensed as a Licensed Practical Nurse (LPN) in your practicing state
• Prior MDS experience is highly preferred
• At least one year of experience in LTC/SNF/AL/MC setting is highly preferred
• Strong knowledge of Case-Mix, Medicare PPS, and Medicaid reimbursement is highly preferred
• Understanding of the Quality Indicator process, OBRA regulations, and Minimum Data Set
• Knowledge of the care planning process
• Professional appearance and behavior
• Excellent communication skills, both written and oral
