Qualifications

• Active Illinois RN or LPN license.
• Long-term care experience preferred.
• 1–2 years of MDS experience preferred.

Responsibilities

• Ensure needed resources are available and quality care is delivered to all residents.
• Coordinate the development, review, and revision of resident assessments and care plans.
• Collaborate with interdisciplinary team members to support resident outcomes.
