Allure of Zion is seeking a dedicated MDS Coordinator (LPN or RN) to join our skilled nursing team in Zion, IL. You will coordinate MDS assessments, collaborate with the interdisciplinary team, and ensure accurate documentation for regulatory compliance and appropriate reimbursement.

Qualifications

Active IL LPN or RN license required.
Experience in long-term care or MDS processes preferred.
Strong attention to detail and regulatory knowledge.

Responsibilities

Coordinate the MDS assessment process for residents.
Ensure timely completion of Medicare/PPS, quarterly, annual, and significant change assessments.
Coordinate interdisciplinary team participation in the MDS process.
Maintain MDS calendar and ARD schedule.
Review documentation to support accurate MDS coding and reimbursement.
Complete MDS data entry and electronic submission.
Verify successful electronic submissions of assessments.
Maintain accurate MDS records and supporting documentation.
Collaborate with nursing, therapy, dietary, social services, activities and others.
Monitor assessment completion to meet deadlines.
Stay updated on CMS/regulatory requirements and participate in care planning.
Promote accurate documentation and quality outcomes.
