Job summary

Jobot is seeking an MDS Coordinator for a long-term care facility in the Kansas City area. You will oversee the MDS assessment process, coordinate with nursing, therapy, social services, and other care team members to ensure timely, accurate data collection and compliant documentation.

The ideal candidate holds an active nursing license or can obtain one and has MDS 3.0 knowledge, RAI processes, and PDPM understanding.

Qualifications

• Active nursing license or ability to obtain one.
• Experience coordinating MDS in skilled nursing or long-term care.
• Solid understanding of RAI, MDS 3.0, and PDPM.
• Strong organizational and time-management skills.
• Excellent communication and teamwork across disciplines.
• Knowledge of federal and state LTC regulations.

Responsibilities

• Coordinate and complete MDS assessments accurately and within required timeframes.
• Manage the RAI/MDS process in accordance with CMS and state regulations.
• Develop and maintain individualized resident care plans in collaboration with the interdisciplinary team.
• Monitor assessment schedules and ensure timely completion of required documentation.
• Review clinical documentation for accuracy, completeness, and appropriate reimbursement.
• Support Medicare and Medicaid reimbursement processes, including PDPM.
• Participate in care plan meetings and communicate with residents, families, and members of the care team.
• Monitor changes in resident condition and ensure assessments and care plans are updated appropriately.
• Maintain compliance with federal, state, and facility standards.
• Assist with audits, surveys, and quality improvement initiatives.
• Serve as a resource to clinical staff regarding MDS documentation and regulatory requirements.
