Job summary

The McKendree Post Acute and Rehab in Hermitage, TN is seeking an MDS Coordinator who is detail-driven and RN or LPN licensed to ensure high-quality resident care. You will lead the MDS/RAI process, coordinate interdisciplinary care planning, and drive accurate assessments and documentation to support regulatory compliance.

You will collaborate with nursing leadership, educate staff on MDS processes, and contribute to quality measures and resident-centered outcomes in a supportive team

Qualifications

• Current LPN or RN license in good standing.
• At least 2 years of MDS experience in a long-term care setting.
• Strong organizational, communication, and assessment skills.
• Commitment to quality care and resident-centered outcomes.

Responsibilities

• Lead and manage the MDS/RAI process from start to finish.
• Coordinate interdisciplinary care planning.
• Ensure accurate and timely completion of assessments and documentation.
