Job summary

Newport Post-Acute and Rehabilitation Center in Newport, TN seeks an RN MDS Coordinator to oversee MDS assessments and related documentation, ensuring accurate coding for appropriate reimbursement.

The role requires an RN license in Tennessee, with preference for long-term care and PDPM/MDS experience. You will collaborate with the interdisciplinary team, lead care plan updates, and support compliance and survey readiness.

Qualifications

• Associate’s degree or higher in nursing (RN).
• Current, unencumbered RN license in Tennessee.
• Long-term Care/ skilled Nursing experience preferred.
• PDPM and MDS experience strongly preferred.

Responsibilities

• Coordinate and complete MDS assessments and related documentation within required timeframes.
• Review medical records and collaborate with the interdisciplinary team to ensure accurate coding and case mix optimization.
• Monitor PDPM and reimbursement impact and communicate findings to facility leadership.
• Assist with care plan development and updates based on assessment findings.
• Educate staff on documentation requirements that support accurate MDS coding.
• Participate in quality improvement and survey-readiness activities related to MDS and documentation.
• Perform other duties as assigned by nursing leadership or the Administrator.
