Job summary

Cane Ridge Post-Acute and Rehabilitation Center in Antioch, TN is seeking an experienced RN MDS Coordinator to oversee the completion and accuracy of MDS assessments and related documentation to support appropriate reimbursement, care planning, and regulatory compliance.

The role requires collaboration with the interdisciplinary team, ongoing PDPM monitoring, and educating staff on documentation standards while assisting with care plan updates and quality improvement activities.

Qualifications

• Current Tennessee RN license (unencumbered).
• Associate degree or higher in nursing.
• Long-term care/skilled nursing experience preferred.
• PDPM and MDS experience strongly preferred.
• Strong attention to detail and analytical skills; knowledge of MDS regulations.

Responsibilities

• Coordinate and complete MDS assessments within required timeframes.
• Review medical records and collaborate with the interdisciplinary team to ensure accurate coding.
• Monitor PDPM and reimbursement impact and communicate findings to facility leadership.
• Assist with care plan development and updates based on assessment findings.
• Educate staff on documentation requirements that support accurate MDS coding.
• Participate in quality improvement and survey-readiness activities.
• Perform other duties as assigned by nursing leadership or the Administrator.
