Job summary

Big Horn Rehabilitation and Care Center in Sheridan, WY is seeking an MDS Coordinator to oversee the RAI process and ensure accurate documentation for regulatory compliance and reimbursement.

As an LPN or RN with MDS experience, you will lead care planning, coordinate interdisciplinary teams, and maintain knowledge of CMS regulations, PDPM, and quality measures while supporting staff education and performance improvement initiatives.

Qualifications

• Active LPN or RN license in Wyoming is required.
• Minimum 2 years nursing experience, with at least 1 year in long-term care.
• Strong knowledge of MDS 3.0, PDPM, RAI guidelines and care planning.
• Familiarity with CMS regulations, Medicare/Medicaid billing and quality measures.
• Excellent organizational, communication, and analytical skills.
• Proficiency in electronic health records (EHR) and MDS software.
• Ability to work independently and collaboratively with cross-functional teams.
• High attention to detail and commitment to accuracy and compliance.

Responsibilities

• Oversee and manage the RAI process including MDS assessments and care planning.
• Ensure timely and accurate MDS submissions in line with CMS and state guidelines.
• Audit documentation to support MDS coding and reimbursement accuracy.
• Lead care plan meetings with the interdisciplinary team.
• Educate nursing staff on documentation best practices and regulations.
• Track resident acuity and case mix data for financial and clinical planning.
• Coordinate with billing, therapy, and reviewers to ensure proper reimbursement.
