Job summary

Briarwood Rehabilitation and Healthcare Center is seeking a dedicated MDS Coordinator to ensure compliant, timely MDS assessments that support resident care planning and reimbursement. This full-time role works with an interdisciplinary team in a 120-bed sub-acute and long-term care facility.

The ideal candidate holds an active RN license, at least 3 years in long-term care, and MDS/RAI experience. Expect collaborative leadership, regulatory focus, and ongoing quality improvement initiatives.

Qualifications

• RN license, active and in good standing.
• Minimum of three years in a long-term care setting.
• Prior MDS/RAI experience required.
• Strong clinical assessment skills with attention to detail.
• Working knowledge of Medicare/Medicaid regulations.

Responsibilities

• Ensure timely, accurate completion of all MDS assessments per regulations and deadlines.
• Supervise MDS data entry, validation, and transmission for accuracy and timeliness.
• Identify, investigate, and resolve data discrepancies and validation issues.
• Prepare and present MDS-related reports to the DON and leadership.
• Assist with audits, surveys, and regulatory reviews and responses.
• Stay current on Medicare/Medicaid regulations and guidelines.
• Collaborate with interdisciplinary teams to accurately reflect resident needs.
