Job summary

Plantation Rehabilitation & Health Center is seeking a full-time MDS Coordinator for our skilled nursing facility in Florida. The role involves leading MDS processes, coordinating assessments, and ensuring timely completion of care plans for Medicare residents.

Ideal candidates will hold an active Florida RN license (BSN preferred) and possess strong communication skills. This position offers a 40-hour workweek with benefits and a team-focused work environment.

Qualifications

• Must hold an active Florida RN license.
• BSN preferred or required as stated.
• Solid communication skills and the ability to read/write in English.

Responsibilities

• Completes an admission care plan for the residents.
• Determines the Assessment Reference Date at time of admission.
• Schedules assessments for all Medicare residents to meet PPS guidelines.
• Conducts full MDS assessment on or before day 5; coordinates Comprehensive Assessment by day 14.
• Notifies Interdisciplinary Team of Care Plan meetings and coordinates the plan.
