Job summary

Sippican Rehabilitation and Healthcare Center seeks an experienced MDS Coordinator (OSHA Classification 1) to lead comprehensive nursing assessments and MDS/RAPS development. You will collaborate with the facility’s interdisciplinary team to support short- and long-term improvement goals and ensure regulatory compliance.

Applicants should be a Connecticut-licensed RN with CPR certification, strong English communication, and proficiency in MS Office. Some overtime and varied scheduling may apply.

Qualifications

• RN licensed in the State of Connecticut.
• Minimum of three (3) years in geriatric nursing or rehabilitation preferred.
• CPR certified.
• Ability to read, write and comprehend English; follow instructions.
• Fluent knowledge of MS Office and related office equipment.

Responsibilities

• Oversee complete MDS coordination and RAI process documentation.
• Lead interdisciplinary care conferences and care planning.
• Ensure timely submission of MDS/RAI for regulatory compliance.
• Coordinate with DNS/ADNS and nursing leadership on PDPM recommendations.
• Maintain confidential resident information and records.
