If you are looking to make adifference in the lives and families that we serve, we are looking for a full-time (40 hours/per week and benefit eligible) MDS Coordinator tojoin our skilled workforce in our skilled nursing facility setting.
TheMDS Coordinator Job Responsibilities:
- Completes an admission care plan for the residents.
- Determines the Assessment Reference Date at time of admission.
- Advices the unit of the “look back” period and provides the unit withthe Certified Nursing Assistant worksheet.
- Schedules assessments for all Medicare residents to meet PPSguidelines, notifies appropriate staff assessment dates and assures thatrequired time frames are met without exception.
- Conducts the full assessment on or before day 5th. Coordinates the completion of the Comprehensive Assessment on or Beforeday 14th.
- Notifies the Interdisciplinary Team of the Resident Care Planmeeting. Coordinates the Care Planmeeting.
- Completes and faxes to the corporate office the Medicare Verificationof Funding form in a timely manner (By Day 5 upon admission and upon RUGSIII level change during stay). Coordinates all RUGS III level changes throughout resident’s stay,working closely with Therapy and Nursing. Communicates all reimbursement level changes to Business OfficeManager as soon as pending level change is identified.
- Identifies potential discrepancies between therapy provides andRUG III claimed on Medicare A residents.
- Runs draft of all MDS’s prior to locking the MDS on all Medicare Aresidents, and assures all MDS's are transmitted within the required time frames.
- Attends and actively participates in all utilization review andtherapy team conference meetings.
- TheManager on Duty will do facility rounds, checking each department and gettingreports from the nursing supervisors. Duties will include, but are not limitedto, facility tours for prospective admissions, assisting with resolution ofresident concerns and overall facility operation.
- All other duties assigned.
Qualified applicants will have the following:
- Must have an active Florida Registered Nurse License, BSN.
- Minimum, three (3) years’ experience in a skilled nursing facility(SNF).
- Point Click Care experience preferred.
- Must know basic data entry, MDS software experience preferred
- Must be able to read, write, speak and understand the English languageand possess good communication skills.
- Must possess the ability to make independent decisions whencircumstances warrant such action.
- Must possess the ability to handle tactfully with personnel, residents,family members, visitors, government agencies/personnel and the general public.
- Must be able to work harmoniously with other personnel.
- Must possess the ability to plan, organize, develop, implement, andinterpret the programs, goals, objectives, policies and procedures, etc. thatare necessary for providing sound accounting techniques.
- Must be able to carry out and understand written and oral instructions.
- Must possess the ability to examine and verify financial documents andreports.
WorkingConditions:
- Works in office areas as well as throughout the facility
- Ability to sit for time required to complete paperwork duties required
- Concrete organizational skills
- Able to work well with staff (Nursing, Therapy, Interdisciplinaryteam members, Business Office)
- Understands clinical skills and documentation
- Able to be mobile which includes lifting up to 20 lbs. of force orforce frequently.
