Socket.dev is seeking an MDS Coordinator to lead assessment coordination, ensure regulatory compliance, and manage MDS documentation for residents. You will work with nursing and therapy teams to optimize care planning and data reporting.

The role emphasizes accuracy in data entry, timely communication, and ongoing professional development to stay current with industry standards and best practices.

Qualifications

• Requires active RN license in good standing.
• Experience with MDS processes and care coordination.
• Strong documentation and communication skills.
• Familiarity with Federal/State regulations governing long-term care.

Responsibilities

• Coordinate resident assessments (admissions, annuals, quarterlies, Medicare).
• Ensure regulatory compliance for MDS and related processes.
• Monitor and organize MDS and care plan documentation.
• Lead interdisciplinary meetings and coordinate care planning.
• Update care plans based on current assessments and resident status.
• Generate and distribute quality indicators reports to leadership.
