Job summary

Avante at St. Cloud Skilled Nursing and Rehabilitation Center seeks an MDS Coordinator to oversee and coordinate resident assessments, ensuring compliance with federal, state, and local regulations.

As a licensed nurse, you will lead the MDS process, collaborate with the Interdisciplinary Care Team, and maintain Five-Star quality measures while protecting resident confidentiality and privacy.

Qualifications

• Active unencumbered nursing license in Florida (RN or LPN).
• Degree/diploma in nursing from an accredited program.
• 2+ years of experience in a hospital, skilled nursing, or healthcare facility.
• Strong knowledge of nursing practices and regulatory guidelines.
• Proven leadership and collaboration with interdisciplinary teams.
• Excellent organizational and critical thinking abilities.
• Compassionate, patient-focused approach.

Responsibilities

• Conduct and coordinate the MDS assessments and care planning to ensure regulatory compliance.
• Ensure timely submission of MDS to the State Repository per RAI guidelines.
• Collaborate with the interdisciplinary care team to set review dates.
• Update resident care plans to reflect health changes and quarterly assessments.
• Monitor Quality Measures and support Five-Star ratings.
• Educate nursing staff, residents, and families on care plans.
• Participate in surveys and regulatory inspections.
• Maintain confidentiality and compliance with Avante policies.
