Avante at Orlando Skilled Nursing and Rehabilitation Center is seeking a Part-Time MDS Coordinator to oversee resident assessments and ensure regulatory compliance. Join a compassionate team dedicated to high-quality patient care and continuous improvement.

Responsibilities include coordinating MDS, submitting assessments on time, and collaborating with the care team to refine care plans while maintaining quality measures and Five-Star ratings.

Qualifications

• Active, unencumbered Licensed Nurse (LPN/RN) in the state.
• Nursing Degree/Diploma from an accredited school.
• 2+ years of experience in a hospital, skilled nursing, or healthcare facility preferred.

Responsibilities

• Conduct and coordinate the MDS assessments and care planning in compliance with all regulations.
• Ensure timely and accurate submission of MDS assessments to the State Repository per RAI Manual guidelines.
• Work with the Interdisciplinary Care Team to set review dates for assessments.
• Evaluate and update resident care plans to reflect health status changes.
• Monitor Quality Measures with focus on maintaining high Five-Star ratings.
• Educate and collaborate with staff, residents, and families on care plans.
