Job summary

The Laurels of Huber Heights seeks an experienced MDS Nurse Coordinator to oversee the RAI process and coordinate care plans for residents. The coordinator supervises the Care Management Nurse and MDS Nurse, ensuring timely assessments and interdisciplinary collaboration.

Qualifications

• Knowledge of the Resident Assessment Instrument (RAI).
• Experience as an MDS Nurse or equivalent in long-term care.

Responsibilities

• Completes the MDS, CAA’s and care plans within regulated time frames.
• Coordinates scheduling the RAI process with the interdisciplinary team.
• Assesses resident through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies for assessment and care planning.
• Coordinates, identifies, and initiates significant changes in MDS.
• Conducts PPS meetings and maintains PPS board for Medicare days and RUGs in absence of Care Management Coordinator.
• Keeps current with AANAC requirements.
