Job summary

Laurel Health Care Company in Ohio is seeking an experienced Care Management Nurse, MDS Nurse, to stay clinically involved in patient care while maintaining autonomy. You will work with the RAI process, conduct assessments, and coordinate care plans for residents at The Laurels of Huber Heights.

The role emphasizes collaboration with physicians, nursing, social services, therapy, dietary, and activity staff; requires RN or LPN licensure and familiarity with PPS and AANAC guidelines.

Qualifications

• Licensed RN or LPN to practice in Ohio.
• Knowledge of the RAI process and PPS requirements.
• Experience as an MDS Nurse.

Responsibilities

• Completes the MDS, CAAs and care plans within regulated time frames.
• Assesses residents through physical assessment, interview and chart review.
• Discusses resident care needs with care givers, including physician, nursing, social services, therapy, dietary, and activity staff.
• Reviews information from hospital, consults and outside agencies for assessment and care planning.
• Coordinates, identifies, and/or initiates significant changes to MDS.
• Maintains PPS board and PPS schedules for Medicare days and RUGs utilization.
• Stays current with AANAC requirements.
