Qualifications

• Bachelor’s Degree in Nursing, Healthcare Administration, or related field required.
• Minimum 2+ years in a leadership role, preferably in a Skilled Nursing Facility.
• Strong MDS knowledge and understanding of assessment and care planning principles.
• Excellent communication with patients, families, and healthcare professionals.
• Proven leadership and management skills with ability to inspire a team.

Responsibilities

• Oversee the entire MDS process, ensuring compliance with federal and state regulations and accrediting standards.
• Develop and implement policies, procedures, and protocols for MDS assessments, care plans, and delivery.
• Conduct thorough MDS assessments of patients' physical, psychological, and social needs to identify improvement opportunities.
• Collaborate with the interdisciplinary team to develop individualized care plans meeting patient needs and goals.
• Ensure high-quality, person-centered care addressing physical, emotional, and social needs.
• Directly supervise and mentor MDS coordinators, therapists, and other healthcare professionals.
