GLENDALE GARDENS NURSING & REHAB in Springfield, MO is seeking an MDS Coordinator to join our team. The role focuses on completing MDS assessments and care plans, initiating care planning, and maintaining monthly calendars while ensuring timely documentation.

The candidate must hold a valid RN or LPN license, have prior MDS and long-term care experience, and be highly organized with excellent communication skills. This role emphasizes confidentiality and teamwork within a clinical setting.

Qualifications

• Valid RN or LPN License.
• Prior MDS experience is required.
• Long-Term Care experience is required.
• Must be able to work 40 hours a week.
• Highly organized, professional and motivated.
• Excellent communication skills.
• Friendly and a team worker.
• Knowledge of skilled nursing facility survey process and regulations would be helpful.

Responsibilities

• Completing accurate assessments, MDS and care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records, ensuring accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident and Facility information at all times.
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.
