Job summary

Nexion Health, Inc. is seeking an MDS Coordinator to lead comprehensive resident assessments (MDS) and ensure data accuracy and timely completion. You will serve as a resource for care planning and orient staff to the MDS process.

Collaboration with an interdisciplinary team, regulatory compliance focus, and reimbursement accuracy will be key. The role is based in Shreveport, LA with a Monday–Friday 8am–5pm schedule. 100% Paid Vacation and Sick/Holiday Time and other benefits are provided.

Qualifications

• Graduate of an accredited nursing school.
• LPN/LVN or RN license required.
• Working knowledge of regulatory and compliance standards related to MDS.

Responsibilities

• Coordinates and completes comprehensive resident assessments (MDS) and ensures accuracy and timeliness of assessment data.
• Serves as a resource for resident care planning activities and conducts orientation sessions on the MDS process.
• Collaborates with the interdisciplinary team on care planning and supports regulatory compliance and reimbursement accuracy.
