Job summary

Atlas Post Acute at Woodbury Country Club is seeking a professional to coordinate the facility's RAI process and ensure accurate MDS assessments and related documentation. The role includes implementing each resident's comprehensive plan of care and scheduling interdisciplinary care plan team meetings.

You will audit medical records for supporting documentation and educate department heads, physicians, and staff as needed.

Qualifications

• Experience coordinating RAI processes with state and federal requirements.
• Accurate completion of MDS assessments and related documentation.
• Ability to implement and evaluate comprehensive care plans.
• Lead and schedule interdisciplinary care plan meetings.
• Audit medical records for supporting documentation.
• Provide education to department heads, physicians and staff as needed.
• Collaborate with health plans and physicians for approvals and eligibility.
• Interpret rules and guidelines for SNF PPS and quality reporting.
