Job summary

Brentwood Place Three is seeking a qualified MDS Coordinator responsible for timely and accurate completion of the RAI process and care management from admission to discharge, aligned with policy, federal/state guidelines, and certifications.

Under direction from Nursing leadership, you will collaborate with the Interdisciplinary Team, schedule MDS assessments, ensure compliant documentation, and maintain MDS status while protecting resident confidentiality.

Qualifications

• Knowledge of MDS 3.0 and RAI guidance.
• Understanding Case-Mix, PPS, and reimbursement processes.
• Familiarity with OBRA regulations and data integrity.

Responsibilities

• Collaborate with the Interdisciplinary Team to assess resident needs.
• Schedule MDS assessments and care plan reviews per guidelines.
• Ensure compliance with RAI, MDS coding and federal/state regs.
• Maintain current MDS status and data entry in MDS systems.
• Attend meetings, communicate changes, and update records.
• Protect resident confidentiality and coordinate care planning.
