Job summary

Southfield Wellness Community is seeking a dedicated MDS Coordinator to join our skilled nursing team. You will lead resident assessments, develop, monitor, audit and modify care plans to meet each resident’s unique needs and goals, and assist in the discharge process.

As an applicant you should hold an unencumbered State of Iowa RN license or LPN certificate, be a graduate of an accredited nursing program, and have CPR certification.

Qualifications

• Unencumbered State of Iowa RN license or LPN certificate.
• Graduate of an accredited nursing program.
• CPR certification required.
• Prior MDS Coordinator experience and 1 year RN SNF experience preferred.

Responsibilities

• Develop, monitor, audit, and modify each resident’s care plan.
• Perform resident assessments and assist in the discharge process.
• Provide knowledge and guidance to the care team to ensure high-quality, individualized care.

As a MDS Coordinator you are instrumental in giving your team the knowledge they need to care for each resident’s unique needs. Your work will ensure our residents receive the high standard of care they have grown to expect at Southfield Wellness Community by developing, monitoring, auditing, and modifying each resident’s care plan for their individual needs and goals, performing resident assessments and assisting in the discharge process. Our residents will depend on your knowledge, skills, and attention to detail to ensure they are comfortable and safe.

To be eligible for consideration applicants should have:

As a minimum, an unencumbered State of Iowa R.N. License or LPN certificate, be a graduate of an accredited nursing program and C.P.R. Certification. Prior experience as a MDS Coordinator, one (1) year of experience as an R.N. in a Skilled Nursing Facility setting and (1) year of experience as a C.N.A. is preferable but we are willing to invest in the right candidate!
