Job summary

Massapequa Center Rehabilitation & Nursing in Amityville seeks an experienced MDS Coordinator to join our interdisciplinary team. This is an excellent opportunity for an RN looking to move into a nonclinical role.

Responsibilities include completing accurate MDS and care plans, monitoring documentation for coding, and maintaining up-to-date assessments. The role emphasizes privacy, professionalism, and collaborative care in a skilled nursing setting.

Qualifications

• Experience with MDS 3.0 is required.
• Valid NY State RN License is required.
• Long Term Care experience is preferred.
• Must be highly organized, professional and eager to learn.
• Strong computer and communication skills.
• Detail oriented and motivated to train with the team.

Responsibilities

• Complete accurate assessments, MDS and care plans as assigned.
• Monitor MDS and care planning documentation for all residents; ensure documentation supports MDS coding in medical records.
• Initiate and support care plans and activities as assigned.
• Maintain and update all care plans and assessments as required.
• Monitor and audit clinical records for accuracy and timeliness.
• Protect the confidentiality of Resident and Facility information at all times.

We are seeking an MDS Coordinator to join our interdisciplinary team of skilled health care professionals at our skilled nursing facility in Amityville!This is an excellent opportunity for an RN looking to move into a nonclinical role.

Job responsibilities include but are not limited to:

• Completing accurate assessments, MDS & care plans as assigned.
• Monitors MDS and care planning documentation for all residents; ensures documentation is present in the medical record to support MDS coding.
• Initiating care plans and supporting activities as assigned.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Protecting the confidentiality of Resident & Facility information at all times.

REQUIREMENTS:

• Experience with MDS 3.0 required.
