Northern Manhattan Rehabilitation and Nursing Center is seeking a detail-oriented and compassionate MDS Registered Nurse (RN) to join our interdisciplinary team. In this role, you will be responsible for coordinating and completing the Minimum Data Set (MDS) assessments to ensure accurate care planning, regulatory compliance, and optimal reimbursement, while supporting high-quality, resident-centered care.

Responsibilities
  • Coordinate and complete MDS assessments in accordance with federal and state regulations
  • Develop and update comprehensive care plans in collaboration with the interdisciplinary team
  • Ensure accuracy and timeliness of all MDS documentation and submissions
  • Monitor clinical documentation to support appropriate case mix and reimbursement
  • Participate in care plan meetings and communicate resident needs effectively
  • Collaborate with nursing, therapy, and administrative staff to ensure regulatory compliance
Qualifications
  • Current Registered Nurse (RN) license in the state of New York
  • Prior experience with MDS assessments in a skilled nursing facility preferred
