Job summary

The Center for Extended Care at Amherst is seeking a dedicated MDS Coordinator to join our clinical team. In this key role, you will oversee MDS assessments and care planning to ensure high-quality resident outcomes and regulatory compliance.

You will collaborate with interdisciplinary teams, maintain EMR records, and support staff development in a family-owned, long-term care setting committed to compassionate rehabilitative and skilled nursing services since 1967.

Qualifications

  • MDS Coordinator certification or equivalent experience.
  • Current RN nursing license is required.
  • Strong knowledge of MDS assessment and care planning.
  • Excellent communication, organizational, and interpersonal skills.
  • Experience with electronic medical records (EMR) or computer-based systems.

Responsibilities

  • Collaborate with interdisciplinary teams to develop and implement individualized care plans.
  • Conduct MDS assessments to identify resident needs and develop care plans.
  • Communicate with residents, families, and healthcare professionals for coordinated care.
  • Maintain accurate records and databases to ensure regulatory compliance.

Job Summary:

The Center for Extended Care at Amherst is seeking a dedicated and compassionate MDS Coordinator to join our team. As a key member of our clinical staff, you will play a vital role in ensuring the highest quality of care for our residents. Our family-owned and operated facility has been dedicated to providing exceptional rehabilitative and skilled nursing services since 1967. Responsibilities:
  • Collaborate with interdisciplinary teams to develop and implement individualized care plans that meet the unique needs of each resident
  • Conduct comprehensive assessments, including Minimum Data Set (MDS) assessments, to identify resident needs and develop effective care plans
  • Communicate with residents, families, and healthcare professionals to ensure seamless care coordination and excellent customer service
  • Maintain accurate and up-to-date records and databases to ensure compliance with regulatory requirements
  • Participate in case conferences and care plan reviews to promote continuity of care and improve resident outcomes
  • Leverage expertise and knowledge to support staff development and training, promoting a culture of excellence and innovation
Requirements:
  • MDS Coordinator certification or experience in a similar role
  • Current RN nursing license
  • Strong understanding of MDS assessment and care planning principles
  • Excellent communication, organizational, and interpersonal skills
  • Ability to work effectively in a fast-paced environment, prioritizing multiple tasks and deadlines
  • Strong analytical and problem-solving skills, with the ability to think critically and creatively
  • Familiarity with electronic medical records and computer-based systems
