Job summary

Northern Metropolitan is seeking an MDS Coordinator and MDS Assessor in Monsey, NY. The role requires completing MDS assessments, care plans, and monthly calendars while maintaining accurate records and confidentiality.

The candidate must have MDS 3.0 experience, a NY RN license, and strong organization and communication skills. This position is focused on long-term care with team-based collaboration.

Qualifications

  • Must have prior MDS 3.0 experience.
  • Valid New York RN license.
  • Long term care experience required.
  • Highly organized, professional and motivated.
  • Solid computer skills and ability to navigate EMR.
  • Excellent communication and team-work abilities.

Responsibilities

  • Complete MDS assessments and care plans as assigned.
  • Initiate and support care plans and related activities.
  • Create and distribute monthly care plan calendars timely.
  • Maintain and update all care plans and assessments as required.
  • Monitor and audit clinical records for accuracy and timeliness.
  • Inform DON of non-compliant documentation issues.
  • Protect confidentiality of resident and facility information.

Northern Metropolitan is hiring a MDS Coordinator and MDS Assessor in Monsey, NY.

Duties
  • Completing accurate assessments, MDS & care plans as assigned
  • Initiating care plans and supporting activities as assigned
  • Creating and distributing monthly care plan calendars in a timely fashion
  • Maintaining & updating all care plans and assessments as required
  • Monitoring & auditing clinical records, ensuring accuracy & timeliness
  • Informing DON of persistent issues related to non-compliant documentation
  • Protecting the confidentiality of Resident & Facility information at all times
Requirements
  • Must Have Prior Mds 3.0 Experience
  • Valid New York RN License
  • Long Term Care Experience Required!
  • Must be highly organized, professional & motivated
  • Should have solid computer skills
  • Excellent communication skills
  • Should be friendly and a team worker
