Qualifications

Administrative functions:
  1. Assist in the collection of resident assessment data (MDS), in accordance with current rules, regulations, and guidelines, including the implementation of CAA and triggers.
  2. Review updates of written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan.
  3. Participate in an ongoing quality assurance program for the collection of resident data and care plans.
  4. Participate in functions involving discharge plans as necessary.
  5. Implement procedures with the RN resident assessment/care plan coordinator to inform all assessment team members of the arrival of newly admitted patients.
  6. Develop the schedule of activities required for completion of the RAI process.
  7. Assist with coordinating the development of a written plan of care (preliminary and comprehensive) for each resident that identifies problems/needs, indicates the care to be given, goals to be accomplished, and responsible professionals.
  8. Conduct daily, concurrent utilization review via chart audit to ensure timely initiation of change of condition RAI’s.
  9. Conduct day concurrent utilization review via chart audits to ensure smooth discharge planning relative to resident’s progress and Medicare coverage.
  10. Maintain the confidentiality of all resident care information.
Qualifications & Experience Requirements for MDS Coordinator:
  • Previous experience as a MDS Coordinator preferred.
  • Licensed Practical Nurse (LPN) or Licensed Registered Nurse (RN) in the State of Connecticut required.
  • Must be an LPN or RN.
