Job summary

Epic Healthcare is seeking an experienced MDS Coordinator to join our team. You will oversee MDS assessments and coordinate care plans to align with regulatory guidelines and reimbursement goals.

The role requires an Active RN or LPN license and hands-on experience with the RAI process in LTC/SNF settings. You will collaborate with an interdisciplinary team and maintain meticulous documentation to support audits and compliance.

Qualifications

• Active RN or LPN license.
• MDS experience in LTC/SNF preferred.
• Strong knowledge of RAI process and care planning.
• Excellent attention to detail and organizational skills.
• Ability to work collaboratively with clinical staff.
