Job summary

Nexion Health Management in Louisiana seeks an experienced MDS Coordinator to lead care planning and documentation. You will coordinate MDS development, assist with PPS requirements, and ensure accurate resident assessments in a skilled nursing setting.

Ideal candidates hold a current RN or LVN/LPN license, have 1+ year of SNF MDS experience, and demonstrate strong communication, organization, and attention to detail. Proficiency with RAI/PPS is essential.

Qualifications

• Must be a graduate of an accredited school of nursing.
• Demonstrated proficiency in RAI and PPS processes.
• 1+ years of experience in skilled nursing facilities with experience completing MDSs and care plans.
• Strong attention to detail, documentation, and resident assessment skills.

Responsibilities
