AVIR Health Group is seeking a qualified MDS Coordinator to join our family in Texas. You will oversee the MDS process, ensuring accurate RAI completion and timely transmission to state agencies, while upholding Federal and State regulations.

We value a resident and family-focused leader who can build and guide a high-performing team and culture. A Texas nursing license and CPR certification are required, with supervisory experience in healthcare.

Qualifications

• Must hold Texas nurse license and CPR certification.
• At least 1 year supervisory experience in healthcare setting.
• Knowledge of long-term care regulations per Federal/State guidelines.

Responsibilities

• Complete the RAI process for each resident as required by State and Federal regulations.
• Transmit and verify receipt by the State.
• Lead and develop a high-performing MDS team and culture.

We are searching for a Licensed MDS Coordinator who is resident & family focused, a team builder, and excited about the opportunity to build and lead a high-performing team and culture. The primary purpose is to complete the RAI process for each resident of the facility, as required by State and Federal regulations. This includes transmission and verification of receipt by the State.

Qualifications for MDS Coordinator:

• Valid Texas Nurse License.
• Valid CPR Certification.
• Minimum of one year of experience in a supervisory capacity in a hospital, skilled nursing facility, or other related healthcare program.
• Must possess knowledge of long-term care operational standards as set forth in the Federal Register, Conditions of Participation and State regulations.
