Job summary

River Crossing Rehab and Healthcare Center is seeking an experienced LPN/RN MDS Nurse Coordinator to join its leadership team. This on-site role focuses on coordinating Resident Assessment Instruments (RAIs) and developing individualized care plans within a supportive management structure.

The ideal candidate will hold an active Missouri LPN or RN license, have prior MDS coordinating experience, and possess a strong understanding of Medicare/Medicaid regulations to ensure regulatory compliance

Qualifications

• Active and current LPN or RN license in Missouri.
• Previous experience as an MDS Coordinator.
• Strong understanding of Medicare/Medicaid regulations.

Responsibilities

• Conduct and coordinate RAIs per federal, state, and local regulations.
• Collaborate with Interdisciplinary Care Plan Team to develop resident-specific care plans.
• Evaluate resident conditions and ensure all special assessments are completed as required.
• Provide data for the Facility's Quality Assurance Program.
