Job summary

RIVER CITY LIVING CENTER in Jefferson City, Missouri is seeking an MDS Coordinator to join our team. You will complete accurate MDS assessments, care plans, and maintain documentation to ensure compliance.

The role requires an RN or LPN license, prior MDS experience, and long-term care knowledge. You must be organized, professional, and capable of working a 40-hour week and collaborating with staff.

Qualifications

• Valid RN or LPN license required.
• Prior MDS experience required.
• Long-term care experience required.
• Must be able to work 40 hours per week.
• Knowledge of skilled nursing facilities survey process helpful.

Responsibilities

• Completing accurate MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records for accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting confidentiality of resident and facility information at all times.
• Participating in meetings and committees such as Medicare, QAPI, Weight Management, etc.

RIVER CITY LIVING CENTER, located in Jefferson City, Missouri is currently seeking an MDS Coordinator to join our team.

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

• Valid RN or LPN License

• Long Term Care Experience Required
