Job summary

Luxor Healthcare is seeking an experienced MDS Coordinator (RN or LPN) in Missouri to join a dedicated skilled nursing facility. You will own the full RAI/MDS process and collaborate with the interdisciplinary team to craft meaningful care plans.

PRIMARY RESPONSIBILITIES include coordinating resident assessments, interviews, and documentation. The role emphasizes accuracy, compliance with CMS/Medicare regulations, and data support for QA initiatives.

Qualifications

• Active RN or LPN license in Missouri.
• Prior MDS Coordinator experience required.
• Strong knowledge of Medicare/Medicaid billing and compliance.

Responsibilities

• Own the full RAI/MDS process from assessment through care plan completion.
• Partner with the Interdisciplinary Team to build individualized care plans.
• Conduct resident interviews and clinical assessments.
• Identify needs for specialized evaluations (behavioral, skin integrity, etc.).
• Ensure timely, accurate documentation and certification of all assessments.
• Contribute data and analysis to support the Quality Assurance Program.

What We're Looking For:

• Active RN or LPN license in Missouri
• Prior MDS Coordinator experience (this is a must)
• Strong working knowledge of Medicare/Medicaid billing and compliance
• Someone who takes ownership and doesn't need to be micromanaged
