REPUBLIC NURSING & REHAB in Republic, Missouri is seeking an RN MDS Coordinator to join our team. The role focuses on completing accurate assessments, MDS and care plans, initiating plans, and maintaining timely documentation to support resident care and compliance.

The ideal candidate holds a valid RN license, has prior MDS experience in long-term care, and is highly organized with strong communication and computer skills.

Qualifications

• RN license required.
• Prior MDS experience in a skilled nursing/long-term care setting preferred.
• Experience with survey processes and regulations helpful.
• 45 hours? Not required; role is 40 hours per week.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records for accuracy and timeliness.
• Informing DON of non-compliant documentation issues.
• Protecting resident and facility confidentiality.
• Participating in Medicare, QAPI, Weight Management committees.

REPUBLIC NURSING & REHAB, located in Republic, Missouri is currently seeking an RN MDS Coordinator to join our team.

MDS Coordinator Duties and Responsibilities:

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion

• Maintaining & updating all care plans and assessments as required

• Monitoring & auditing clinical records, ensuring accuracy & timeliness

• Informing DON of persistent issues related to non-compliant documentation

• Protecting the confidentiality of Resident & Facility information at all times

• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

MDS Coordinator Qualifications and Skills:

• Valid RN License

• Knowledge of skilled nursing facilities survey process and regulations would be helpful.

• Must have prior MDS Experience

• Long Term Care Experience Required

• Must be able to work 40 hours a week

• Must be highly organized, professional & motivated

• Should have solid computer skills

• Excellent communication skills

• Should be friendly and a team worker
