Job summary

GRANDVIEW HEALTHCARE CENTER in Washington, Missouri is seeking an MDS Coordinator to join our team. The role requires RN/LPN licensure with MDS and long-term care experience.

You will manage MDS assessments, care plans, and documentation while coordinating with the DON and care staff. Strong communication and computer skills are essential, and the position is full-time.

Qualifications

• Valid RN or LPN license.
• Knowledge of skilled nursing facilities survey process and regulations would be helpful.
• Must have prior MDS experience.
• Long Term Care experience required.
• Must be able to work 40 hours a week.
• Must be highly organized, professional & motivated.
• Should have solid computer skills.
• Excellent communication skills.
• Should be friendly and a team worker.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned
• Initiating care plans and supporting activities as assigned
• Creating and distributing monthly care plan calendars in a timely fashion
• Maintaining & updating all care plans and assessments as required
• Monitoring & auditing clinical records, ensuring accuracy & timeliness
• Informing DON of persistent issues related to non-compliant documentation
• Protecting the confidentiality of Resident & Facility information at all times
• Participates in various meetings and committees: Medicare, QAPI, Weight Management etc.

• Completing accurate assessments, MDS & care plans as assigned

• Initiating care plans and supporting activities as assigned

• Creating and distributing monthly care plan calendars in a timely fashion
