Serenity Spring Senior Living at Jasper Rehabilitation & Nursing Center in Jasper, IN, is seeking a Full-Time Registered Nurse Assessment Coordinator (RNAC) to join our healthcare team. The RNAC will assess and document patient needs, coordinate care plans, and ensure regulatory compliance.

Responsibilities include comprehensive patient assessments, developing care plans with clinicians, completing MDS, and ensuring accurate records.

Qualifications

• Current RN license in Pennsylvania.
• Minimum of one year of long-term care or similar healthcare experience.
• Strong knowledge of MDS assessments and Medicare/Medicaid regulations.
• Excellent communication and interpersonal skills.
• Detail oriented with accuracy and compliance.
• Ability to work collaboratively in a team-based environment.
• Proficient with electronic health records (EHR).
• Dedication to high-quality patient care and outcomes.
