Qualifications

Description

Join our fun and growing team! Paradigm Healthcare at Woodwind Lakes is seeking a motivated LVN/RN to be our MDS Coordinator.

Job Duties
  1. Maintain the current MDS status of residents, complying with federal and state laws, regulations, and facility policies.
  2. Schedule assessments (OBRA, Medicare PPS, Medicaid) and communicate schedules to departments and families.
  3. Monitor clinical systems for resident condition changes, coordinate reassessments, and update care plans accordingly.
  4. Set assessment reference dates (ARD) with the interdisciplinary team to accurately reflect resident needs.
  5. Complete MDS and CAAs sections accurately per policies and guidelines.
  6. Participate in daily Casemix Meetings, weekly Utilization Review Meetings, and monthly Triple Check Meetings.
  7. Maintain assessments in the clinical record for at least 15 months as per regulations.
  8. Transmit MDS data electronically to the state following regulations and standards.
  9. Follow facility policies governing nursing services.
  10. Participate in facility surveys and inspections by government agencies.
  11. Contribute to developing and revising resident care plans to maximize wellbeing.
  12. Report any unauthorized access attempts to the facility’s information system.
  13. Educate residents, families, and caregivers as needed or directed.
  14. Provide emergency support, including CPR, following physician orders and policies.

Qualifications

Education and Experience

  • Active, unencumbered license as LVN or RN in the state; RN license preferred.
  • 2+ years of Long-Term Care experience preferred.
  • 6+ months in rehabilitative/restorative nursing preferred.
  • 2+ years in RAI process preferred.
  • Ability to make independent decisions and demonstrate leadership and organizational skills.
  • Knowledge of Point Click Care is a plus.
  • Proficiency in Microsoft Outlook, Office, and Internet.
  • Ability to handle multiple tasks under pressure and meet deadlines.
