Description
Join our fun and growing team! Paradigm Healthcare at Woodwind Lakes is seeking a motivated LVN/RN to be our MDS Coordinator.
Job Duties
- Maintain the current MDS status of residents, complying with federal and state laws, regulations, and facility policies.
- Schedule assessments (OBRA, Medicare PPS, Medicaid) and communicate schedules to departments and families.
- Monitor clinical systems for resident condition changes, coordinate reassessments, and update care plans accordingly.
- Set assessment reference dates (ARD) with the interdisciplinary team to accurately reflect resident needs.
- Complete MDS and CAAs sections accurately per policies and guidelines.
- Participate in daily Casemix Meetings, weekly Utilization Review Meetings, and monthly Triple Check Meetings.
- Maintain assessments in the clinical record for at least 15 months as per regulations.
- Transmit MDS data electronically to the state following regulations and standards.
- Follow facility policies governing nursing services.
- Participate in facility surveys and inspections by government agencies.
- Contribute to developing and revising resident care plans to maximize wellbeing.
- Report any unauthorized access attempts to the facility’s information system.
- Educate residents, families, and caregivers as needed or directed.
- Provide emergency support, including CPR, following physician orders and policies.

Qualifications
Education and Experience
- Active, unencumbered license as LVN or RN in the state; RN license preferred.
- 2+ years of Long-Term Care experience preferred.
- 6+ months in rehabilitative/restorative nursing preferred.
- 2+ years in RAI process preferred.
- Ability to make independent decisions and demonstrate leadership and organizational skills.
- Knowledge of Point Click Care is a plus.
- Proficiency in Microsoft Outlook, Office, and Internet.
- Ability to handle multiple tasks under pressure and meet deadlines.
