Position Summary

The MDS Coordinator assists the Director of Nursing in ensuring documentation compliance with federal, state, and certification guidelines. They coordinate the RAI process, ensuring the timeliness and completeness of MDS, CAAS, and Care Plans.

Qualifications
  • Current LVN license
  • At least two (2) years of nursing experience in a skilled nursing facility preferred
  • Strong knowledge of RAI, Medicare PPS, and Medicaid reimbursement
  • Understanding of Quality Indicator processes, OBRA regulations, and MDS
  • Knowledge of the care plan process
Essential Functions
  • Ensure resident safety and regulatory compliance
  • Participate in surveys and address deficiencies
  • Report audit trends to the Quality Assurance Committee
  • Manage the RAI process from MDS to care plan completion
  • Monitor and track RAI, discharge, re-entry, and Medicaid forms
  • Ensure proper documentation and signatures
  • Coordinate scheduling and communication for care planning
  • Support accurate coding and timely submission of assessments
  • Maintain confidentiality and stay current on industry changes
  • Assist with audits and reviews as needed
  • Utilize electronic record systems effectively
Physical Requirements

Ability to safely perform movements such as pushing, pulling, lifting, bending, and reaching, up to 50 pounds with or without accommodations.
