Qualifications

We are adding to our team and hiring an MDS Coordinator. The primary responsibility of this role is to coordinate and oversee the timely and accurate completion of Resident Assessment Instruments (RAIs) in compliance with state and federal regulations. This includes facilitating interdisciplinary care planning, ensuring proper Medicare utilization review, and mentoring staff to maintain a high standard of care and documentation.

We are seeking an experienced and eager MDS Coordinator for our existing team.

Essential Functions of the Job:

  1. Coordinates all required Resident Assessment Instruments (RAI) per state and federal regulations to ensure timely completion.
  2. Timely, accurate completion of all aspects of nursing portions of required RAIs.
  3. Timely transmission of all required RAIs per state and federal regulations.
  4. Maintenance of an RAI calendar, accessible to members of the Interdisciplinary Team (IDT), which provides clear, concise, up-to-date information on completion deadlines for each phase of the RAI process for each applicable resident.
  5. Monitoring of ancillary department sections of the RAI for timely completion, including notification of the individual departments if deadlines are in danger of not being met by the departments.
  6. Mentoring and teaching nursing and ancillary staff to ensure that all team members are aware of and competent in their roles in the RAI and Prospective Payment System (PPS) processes.
  7. Maintaining smooth, seamless, timely communication with billing, regarding residents’ Medicare status and RUG categories.
  8. Notification to the Charge Nurse of any barriers to timely, accurate completion and transmission of RAIs to prevent missing any deadlines.

Responsibilities to Care Planning:

  1. Coordinate and facilitate interdisciplinary Resident Care Plan Conferences on or about the 10th day of admission for each resident, ensuring that the resident and responsible party receive written invitation and notification of the meeting, specifying time and place.
  2. Coordinate and facilitate interdisciplinary care plans as situations occur on a day-to-day basis.
  3. Actively participate in the development of a comprehensive, individualized, interdisciplinary plan of care for each resident.
  4. Ensure that care plans are completed, updated, and accelerated to reflect an interdisciplinary approach to the residents’ changing needs.
  5. Review each Medicare resident’s case on the day of admission to ensure appropriateness of Medicare coverage.
  6. Active interaction with IDT to evaluate residents’ status and needs relative to Medicare skilled needs and potential for RUG category changes.
  7. Conduct daily, concurrent utilization review via chart audit to ensure that residents receive the care and services needed in a timely manner.
  8. Initiate change of condition RAIs when appropriate.
  9. Ensure chart documentation, including nursing and ancillary progress notes, care plans, and notes reflect the need for the care and services the resident receives.
  10. Ensure discharge planning proceeds smoothly relative to residents’ progress and Medicare coverage status.
  11. Discontinue Medicare coverage when residents no longer meet criteria.
  12. Consult routinely with the Clinical Case Manager assigned to each resident regarding the resident’s Medicare coverage status.
  13. Ensure that Medicare certification and re-certification forms are signed by the physician per regulations.

Qualifications

Education and/or Experience

  • Nursing Degree from an accredited college or university, or completion of an approved RN program.
  • Current, active, and unencumbered RN license in the state of Missouri.

Certificates, Licenses, and Registrations

  • Current, active, and unencumbered RN license in Missouri.
  • Current CPR certification.

Preferred Qualifications

  • Knowledge and experience with PCC (PointClickCare).
  • Maintenance of all required continuing education and licensing.
  • Good standing with the State Board of Nursing always.
