We are seeking a dedicated MDS/Care Plan Coordinator to join our team at Rancho Rehabilitation and Healthcare, a 120-bed facility in Florissant, MO. Our mission is to provide exemplary rehab and skilled nursing services, ensuring high-quality care and positive outcomes for our residents.

• Type: Full-time

Compensation

Base pay range: $68,000.00/yr - $78,000.00/yr. Actual pay will depend on skills and experience. Please discuss with your recruiter for more details.

Responsibilities

• Develop comprehensive, individualized care plans for each resident.
• Ensure care plans are completed, updated, and reviewed per RAI guidelines and resident needs.
• Manage the implementation of Clinical and Prospective Payment System MDS schedules.
• Coordinate resident assessments for reimbursement purposes.
• Oversee all functions of the MDS assessment process to ensure compliance with federal and state regulations.
• Participate in pre-admission and admission care level assessments.
• Track and schedule timely MDS assessments based on resident care changes.
• Review and certify MDS assessments and related documentation.
• Analyze quality indicator reports and review care plans accordingly.
• Maintain current knowledge of relevant regulations regarding RAI, care planning, and PPS.
