Are you a detail-oriented RN with a passion for quality care? Luxor Healthcare is seeking a skilled MDS Coordinator to lead the assessment and care planning process for our residents. If you’re an experienced professional who thrives in a collaborative environment and values excellence, we’d love to hear from you!
Oversee and coordinate the Resident Assessment Instrument (RAI) process in full compliance with current regulations and facility guidelines.
Collaborate with the Interdisciplinary Care Plan Team to develop personalized and comprehensive care plans for each resident.
Conduct and/or coordinate resident interviews as part of the assessment process.
Assess residents’ medical conditions and identify needs for specialized evaluations (e.g., Norton Scale, behavior management, psychotropic medication usage).
Complete accurate, timely preliminary and full assessments, using approved tools and documentation.
Ensure every section of the RAI is properly signed and dated by the responsible staff member.
Certify the completion of assessments with your signature and date.
Provide data and insight to support the facility’s Quality Assurance Program.
Current active RN license in Missouri
Prior experience as an MDS Coordinator
Solid knowledge of Medicare/Medicaid regulations and benefit guidelines
