Job summary

Pearl Healthcare in Crystal Lake, IL is seeking a compassionate MDS Coordinator (RN or LPN) for a 32-hour-per-week role. The candidate will lead the MDS process, coordinate interdisciplinary care and ensure timely assessments.

You will mentor staff, maintain compliant documentation, and collaborate across departments to optimize resident care and regulatory compliance.

Qualifications

• Three years in long‑term or acute health care preferred.
• Strong knowledge of the MDS process.
• Strong knowledge of Medicare codes and SNF guidelines.
• Excellent verbal, organizational and communication skills.
• Strong computer skills.

Job description

*32 hours per week* RN or LPN

Position Responsibilities:

• Embody the company standard as a high‑quality care provider.
• Demonstrate strong leadership.
• Assesses and determine the health status and level of care of all new admissions.
• Ensure the accurate and timely completion of all MDS Assessments including Medicare, quarterly, annual and significant changes.
• Communicate the level of care for new resident to all disciplines.
• Coordinate the interdisciplinary participation in completing the Minimum Data Set (MDS) for each new admission to facility according to regulatory time frames.
• Ensure completeness and thoroughness of documentation as mandated by federal, state and medical standards.
• Maintain an accurate schedule of all MDS assessments to include the proper reference dates throughout the resident’s stay.
• Responsible for the data entry function to assure accurate data entry and electronic submission of MDS assessments.
• Verify electronic submissions of MDS and perform corrections when necessary.
• Coordinate interdisciplinary participation in completing the MDS for each resident according to regulatory time frames.
• Ensure completeness and thoroughness of documentation as mandated by federal and state standards.
• Schedule and conducts resident care conferences in compliance with state and federal regulations.
• Ensure completion of all MDS reviews prior to the resident care conference.
• Assist disciplines in formulating and revising care plans.
• Evaluate resident care plans for comprehensiveness and individuality
• Ensure that resident’s present/potential problems are identified and prioritized; realistic goals are established and nursing intervention is appropriate.
• Manage all level of care changes within the facility. Notify all departments when a level of care change has been made.
• Transmit any required forms to the appropriate agency for processing as required by federal and state law.
• Adhere to the budget guidelines for the department.
• Performs other duties as assigned.

Educational Requirements

Currently licensed as RN in this state; Bachelor's Degree in Nursing preferred.
