Job summary

Martine Center in White Plains, NY is seeking an MDS Coordinator to manage assessments and care plans for our 200-bed rehabilitation and skilled nursing facility.

The role requires a valid New York RN license and MDS 3.0 experience, with strong organizational and communication skills. You will work as part of a team to maintain accurate documentation and ensure high-quality resident care.

Qualifications

• Must have MDS 3.0 experience.
• Valid New York RN license required.
• Long term care experience preferred.
• Excellent communication and organization skills.
• Solid computer skills and teamwork ability.

Responsibilities

• Completing accurate assessments, MDS & care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars in a timely fashion.
• Maintaining & updating all care plans and assessments as required.
• Monitoring & auditing clinical records, ensuring accuracy & timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident & Facility information at all times.
