Job summary

Triboro Center is seeking an MDS Assessor to work in-person at its Skilled Nursing Facility in Bronx, NY. The position requires a valid New York RN license and MDS 3.0 experience.

The ideal candidate will be highly organized, experienced in long-term care, and proficient with computer systems. Responsibilities include completing accurate assessments and care plans, initiating and updating plans, and ensuring timely, compliant documentation. This is an in-house role with the Triboro Center team.

Qualifications

• Valid New York RN license required.
• Experience in Long-Term Care is required.
• Must be highly organized, professional and motivated.
• Solid computer skills needed.
• Excellent communication and teamwork.
• In-person, Bronx, NY facility role.

Responsibilities

• Completing accurate assessments, MDS and care plans as assigned.
• Initiating care plans and supporting activities as assigned.
• Creating and distributing monthly care plan calendars promptly.
• Maintaining and updating all care plans and assessments as required.
• Monitoring and auditing clinical records for accuracy and timeliness.
• Informing DON of persistent issues related to non-compliant documentation.
• Protecting the confidentiality of Resident and Facility information at all times.
